Using TV speakers and Avantree headphones at the same time is mainly a TV feature. Most modern TVs can continue playing through their built-in speakers while also sending audio through Optical Out, but some TVs cannot.

Connect your Avantree transmitter to the TV's Optical / Digital Audio Out, then configure the TV to keep its speakers enabled.
LG TV
LG TVs typically provide a direct simultaneous-output setting.
Once selected, the TV speakers and Optical Out can play at the same time.

These TVs may support simultaneous TV speaker and Optical audio when configured correctly.
Some Samsung models also have Simultaneous Optical Output under Settings → Accessibility. Make sure it is turned ON if available.
Some newer smart TVs can play through their built-in speakers while also sending Bluetooth audio directly to headphones.

Some newer Samsung TVs include a Multi-Output Audio feature.

Compatible LG TVs may provide a Multi-Output Audio or equivalent simultaneous-output option.

Connect the TV to your soundbar or AV receiver, then use an available audio output from that device for the Avantree transmitter.
Pair an additional compatible headphone to your Avantree system so another listener can hear clearly without relying on the TV's built-in speakers.
